I want to take a moment to personally thank you for trusting me with your care. It’s an honor to serve you, and I’m committed to continuing to provide the highest quality of support, service and care.

To ensure we can maintain the level of personalized care and access you’ve come to expect, there will be a small price adjustment to our services effective October 1^st. This change reflects increased cost of operation and continued improvements to enhance your experience. Our functional medicine style will include: traditional primary care, providing a concierge approach that allows rapid access to same day or next day care with quick and unlimited office visits, video visits, phone visits, text, and portal messaging on a monthly basis directly from a single MD. No nurse practitioners or physicians assistants involved.

       Adults 40 and over-                                              $155 monthly

        Spouse membership-                                                  15% off age bracket  

       39 and under -                                                       $85 Monthly

       Children 25 and under-                                          $0

                          $25 co-pay per visit and $25 medication (children)

• Additional Services Offered:

  Ketamine Therapy-                                              $100 Per Session                         (2 hr. session) *pt. will purchase ketamine from compounding pharmacy

       Pellets-                                                                 $375 every 12 weeks

 Boosts-                                                                 $0

Pap Smears-                                                        $25

Pelvic Floor Therapy-  45 min appt                       $75 per session    

Medrol/Allergy shots-                                            $20

We would also like to remind our patients that our business hours are Monday to Thursday 8am to 5pm and Friday 8am to noon. Our clinic will only operate during these designated office hours. Phone calls, text, portal messages and emails will be answered during regular business hours. If you leave a message, please allow up to 24 hours for a response during the week. On weekends and holidays, response time may take 48 to 72 hours.  If you are experiencing a medical emergency, please dial 9-1-1 or proceed to the nearest urgent care or emergency room immediately.

As a reminder for refills, contact your pharmacy first and then reach out to our office.
